Before your first appointment

Please provide your insurance information so benefits can be reviewed. You are encouraged to contact your insurance company directly to confirm:

  • Mental health benefits

  • Telehealth coverage

  • Copay, coinsurance, or deductible

  • Authorization requirements

Clients are responsible for any charges not covered by insurance.

Employee Assistance Program Benefits

EAP benefits may cover a limited number of counseling sessions at no cost to you. Authorization is usually required before the first appointment.

Please provide your EAP authorization number and the number of approved sessions when scheduling.
